Apply for a Service Dog
Service dogs and therapeutic programs are provided to eligible applicants at no cost. Applicants are not required to participate in fundraising or public relations activities without their expressed and voluntary permission. While Got Your Six Support Dogs provides service dogs, training, therapeutic programs, hotel lodging, and most meals free of charge, applicants need to plan for expenses associated with transportation, food, and entertainment costs while attending the training course.
Eligibility Criteria
Although other information may be required, our basic criteria includes:
- United States military or first-responder service
- Verifiable diagnosis of PTSD, TBI, and/or MST which must be service-related
- Have a referral or recommendation, and the support of your healthcare provider for obtaining a service dog
- Verifiable participative sessions with a therapist, psychologist, or social worker on a monthly basis (at minimum) for at least six months
- No alcohol or substance abuse and no illegal dependency
- No felony conviction, pending criminal charges, current parole/probation, pattern of criminality, or animal abuse
- Residence in the territorial United States and no intention to leave for 60 days after graduation from our program
- Ability to travel to our location in Maryville, Illinois and participate fully in our program (including therapies) for 14 days without a caregiver or assistance in activities for daily living
- Must be dedicated to maintaining the dog’s training throughout the life of the team and can financially and physically provide for the well-being of the dog. These costs may include, but are not limited to: food, grooming, toys, other supplies, annual veterinary exam, vaccinations, and other incidental expenses.
Application Process
Please note the waiting time to receive a service dog is currently 1-2 years.
Existing Got Your Six Support Dogs clients who have applied for a successor dog are given priority on the waiting list provided they are in good standing and continue to meet applicable eligibility requirements.

All information provided is privileged and will be considered confidential. Applicants have 60 days to submit their completed application along with all required documents, after which we welcome anyone with an incomplete application who would still like to apply to do so by restarting the process.
Once we’ve received your completed application, Got Your Six Support Dogs will send you a link via email to submit to a background check. Once the background check is completed, we will contact you within 30 days to schedule a virtual or in-person interview.
Shortly after the interview, we will schedule a home inspection that will be conducted in-person or virtually. Once the inspection and interviews are complete, Got Your Six Support Dogs will notify you within three months if you are selected for a service dog placement.
